Job Description Job description: Sign-On Bonus: $5,000 for qualified candidates Location: Clay County | Home Health (Field-Based) Job Summary We are seeking a motivated and compassionate Full-Time Physical Therapist (PT) to provide high-quality home health services to patients throughout Clay County. This role offers autonomy, flexible scheduling, and the opportunity to make a meaningful impact by helping patients regain mobility, function, and independence in their homes. The ideal candidate is clinically strong, patient-focused, and comfortable working independently while collaborating with an interdisciplinary care team. Key Responsibilities · Perform comprehensive physical therapy evaluations and develop individualized treatment plans · Provide skilled PT interventions to improve strength, balance, mobility, and functional independence · Educate patients and caregivers on home exercise programs, safety, and fall prevention · Communicate patient progress and changes in condition to physicians and care team members · Complete accurate, timely documentation in compliance with regulatory and agency standards · Participate in case conferences and interdisciplinary collaboration as needed Qualifications · Active Physical Therapist (PT) license in the state of Florida · Home health experience preferred (new to home health candidates with strong clinical background considered) · Strong clinical reasoning, time management, and communication skills · Ability to work independently in the field · Valid driver’s license and reliable transportation Position Details · Job Type: Full-time · Coverage Area: Clay County · Work Setting: Patient homes Compensation & Benefits · Competitive pay structure · $5,000 sign-on bonus for qualified candidates · Mileage reimbursement · Paid orientation and training · Health, dental, vision, and life insurance · 401(k) with employer match · Paid time off (PTO) · Referral bonus program In accordance with House Bill 531 (2025), candidates for this position are subject to background screening through the Florida Care Provider Background Screening Clearinghouse.
